Notice for Volunteer/Employee Application Form

The Arch/Diocese/Eparchy and the Parish you have elected to volunteer or work in is committed to ensuring the security and safety of its parishes and its members. Our volunteers and employees play an important role in fostering a safe and secure environment.

The Arch/Diocese/Eparchy participates in the usage of the Catholic Mutual Group Safe Environment Platform (the Platform), a database and safe environment training platform to assist with the screening of volunteers and employees at locations across the Arch/Diocese. We use the Platform as part of our on-going risk mitigation efforts to ensure the security and safety of our parish and its members. The Platform is used to safeguard children and vulnerable adults from those who may be motivated to harm those entrusted to the care of the Arch/Diocese/Eparchy.

By completing this Application, you consent:

• To the upload and storage of your personal information, which may include your full name, address, email address, date of birth, telephone number, primary parish or other location, training history, and whether or not you were accepted for this position. Your information will be retained within the Platform for as long as reasonably necessary to fulfill the purposes outlined above.

• To your personal information being referenced against the existing information within the Platform, with the Parish and Arch/Diocese/Eparchy being made aware of whether or not you have applied for and were rejected, or have previously or currently a volunteer or employment position in this Arch/Diocese/Eparchy.

• To your personal information being cross-referenced against information provided by others who apply for a volunteer or employment position at this Arch/Diocese/Eparchy. If records match representatives of the Arch/Diocese/Eparchy will be so informed, and these representatives may initiate further investigation which may include contacting you for further information.

The Platform is secured and your information will be accessed only by those who have been granted authorization and a password. The Arch/Diocese/Eparchy will remain the custodian of your personal information; however, your personal information will be transferred to third party service providers for processing. Some of these service providers may be located outside of Canada, including in the United States, and your personal information may be collected, used, disclosed, stored and processed in these jurisdictions for the purposes described in this notice. By using the Platform, you consent to the transfer or access of information outside the country you live in. While your personal information is located outside Canada, it will be subject to the laws of the jurisdiction in which it is retained, including laws that may require disclosure of personal information to foreign government authorities.
